Propecia (finasteride) was the first oral drug licensed for the treatment of Male Pattern Baldness. It is not a hormone treatment, but a type II 5 alpha reductase inhibitor. This means that it drastically reduces the effect of the enzyme 5 alpha reductase (type II), and in doing so, drastically reduces the conversion of testosterone to dihydrotestosterone (DHT), which it is responsible for in the hair cell. It is DHT that is one of the major culprits for hair loss in genetically determined hair cells. It will not have any effect on hair production where the follicle has died. To maintain any hair regrowth, continued use is necessary. This therapy is not suitable for females, and male patients must thoroughly read the instructions that accompany it. For those patients wishing to obtain optimum results, it is essential to use a type I and type II 5 alpha reductase inhibitor (oral finasteride + topical trichagen lotion).
